The first 50 registrants will receive a free online Health Risk Assessment, to be completed at Advocate Condell Medical Center’s Centre Clubs, located in Libertyville and Gurnee. The confidential screening tool asks for basic health information, provides a detailed report on your health risks and outlines steps that can be taken to reduce your risks and lead a healthier life. The assessment takes about 20 minutes, and participants will receive a complimentary one-week membership to the Centre Club.

The goal of the program is to provide comprehensive support to those who wish to lose weight, exercise or eat healthier in 2012, according to DeBruler.
